Meet the Preceptors at HCA Florida Northside Hospital
Stephanie Johnson, PharmD, BCEMP
PGY1 Residency Program Director
Clinical Pharmacy Specialist - Emergency Medicine
Stephanie is the Emergency Medicine Clinical Pharmacist at HCA Florida Northside Hospital. She graduated with her Doctorate of Pharmacy from the University of Florida in Gainesville, Florida. She completed both a PGY1 Pharmacy Practice Residency and a PGY2 Emergency Medicine Residency at Tampa General Hospital in Tampa, Florida. Following residency, Stephanie joined the HCA Florida Northside Hospital team to pilot the emergency medicine pharmacy services. She serves as a preceptor for APPE pharmacy students and precepts the Emergency Medicine I and Emergency Medicine II rotations within the residency program. Her professional interest areas include trauma resuscitation, anticoagulation reversal, toxicology, and precepting. Stephanie is an active member of the American College of Clinical Pharmacy. In her free time, Stephanie enjoys traveling, hiking, baking, and spending time with her family and friends.Larry Weiss, PharmD, CPh, BCCCP, BCPS
Director of Pharmacy
Larry is the Director of Pharmacy at HCA Florida Northside Hospital. Larry has a BS in Food Science and Human Nutrition from the University of Florida. He earned his Doctor of Pharmacy degree from Virginia Commonwealth University in 2009 and subsequently completed a PGY-1 pharmacy residency in 2010 at AdventHealth Orlando. Larry has been a pharmacist for 13 years, and has 5 years of pharmacy leadership experience. He is an active member of Florida Society of Health-System Pharmacists (FSHP) Southwest Society and the American Society of Health System Pharmacists. In his free time Larry enjoys travel and spending time outdoors.
Jerzi Bauza, PharmD
Pharmacy Clinical Coordinator
Jerzi spent his late childhood and teenage years in Saint Petersburg, Florida. After completing a bachelors in biomedical science at the University of South Florida, he earned his doctor of pharmacy degree from Lake Erie College of Osteopathic Medicine (LECOM) in Bradenton. Throughout his years of academia Jerzi developed a passion for biochemistry, molecular biology, and molecular cancer as he tutored these subjects during his four years at LECOM. Following graduation, Jerzi worked for a retail pharmacy as a lead pharmacist where he developed and acquired excellent patient care skills and great pharmacy management experience. He then transitioned to HCA Florida Northside Hospital where he has filled multiple operational and clinical roles. He currently is the pharmacy clinical coordinator and has joined the West Florida Division Antibiotic Stewardship Committee where his main focus is to ensure an appropriate process for antibiotic stewardship is in place throughout the West Division facilities. He has great aspirations and continues to advance in the profession of pharmacy as he is prepares to take board certification in the near future.
Brad Hoffman, PharmD, BCPS
Clinical Pharmacy Specialist – Critical Care
Brad is a Clinical Pharmacist at HCA Florida Northside Hospital. Brad was born and raised in Jupiter, Florida. He completed his undergraduate studies at Florida State University and received his PharmD at Auburn University. Brad completed his PGY1 pharmacy residency here at HCA Florida Northside Hospital. Brad’s primary interest in pharmacy is critical care and he currently serves as the critical care pharmacist where he rounds with both our cardiothoracic surgery and intensivist teams. In his free time, Brad enjoys watching college football and working on his cars.
Hanna Lefebo, PharmD
Clinical Pharmacist
Hanna is a clinical pharmacist at HCA Florida Northside Hospital. She attended Temple University for her undergraduate studies, where she received her Bachelor’s degree in Neuroscience: Cellular and Molecular in 2018. She then earned her Doctor of Pharmacy degree in 2022 from the University of Maryland School of Pharmacy. Dr. Lefebo completed her PGY1 Pharmacy Residency at Duke Regional Hospital. She also obtained her Residency Teaching Certificate through the Duke Health System. Her professional interests include internal medicine and infectious diseases. Her professional goals are to serve as an Associate Professor to pharmacy students and grow into an administrative role. In her free time, she enjoys activities such as running, weight lifting and hanging out with her friends and family.
Parth Saraiya, PharmD
Clinical Pharmacy Specialist – Internal Medicine
Parth is a clinical pharmacy specialist at HCA Florida Northside Hospital. Parth was born and raised in North Brunswick, NJ. He received his undergraduate degree at Rutgers University and PharmD at Temple University School of Pharmacy. Parth completed his PGY-1 pharmacy residency here at HCA Florida Northside Hospital. Following residency, Parth stayed at Northside as an evening pharmacist before transitioning into the internal medicine position. As the internal medicine pharmacist, Parth is responsible for rounding with the internal medicine team on a daily basis. Outside of pharmacy, Parth enjoys spending time with friends and family, traveling to new places, and rooting for his favorite sports teams, the New York Yankees and Philadelphia Eagles.
Ernie Vargas, PharmD
Clinical Pharmacist
Ernie is a Clinical Pharmacist at HCA Florida Northside Hospital. He was born and raised in Sarasota, FL. He got his bachelors of science in Biology from the University of South Florida and his PharmD from Rosalind Franklin University. Following pharmacy school, Ernie worked full-time at Walgreens before accepting a position with the HCA Florida Blake Hospital team as an overnight pharmacist. He joined the HCA Florida Northside Hospital team as an evening quarterback pharmacist. Currently, he is helping pilot the pharmacy-led medication reconciliation process at Northside. In his free time, Ernie enjoys spending time with his family, playing with his dogs, and playing golf.
Stephen Zambito, PharmD
Pharmacy Operations Coordinator
Stephen is the Pharmacy Operations Coordinator at HCA Florida Northside Hospital, with a strong background in clinical pharmacy management and operational leadership. He is a graduate from the University of South Florida Taneja College of Pharmacy with a concentration in Pharmacy Health and Education. He has led clinical initiatives and is an active member in medication diversion efforts within the hospital. He contributes to the development of pharmacy residents and students as the Pharmacy Staffing Preceptor. Known for his collaborative approach, he consistently supports institutional goals by optimizing pharmacy services and fostering cross-functional relationships across clinical departments.

Brooke Irlacher, PharmD
Brooke is a current PGY1 resident at HCA Florida Northside Hospital. She is originally from Cherry Valley, Illinois. Brooke completed her undergraduate coursework at Southern Illinois University Edwardsville and earned her Doctor of Pharmacy degree from the University of Illinois Chicago College of Pharmacy in Rockford, IL. She has a strong interest in general inpatient clinical pharmacy, where she enjoys the collaborative environment and the opportunity to be closely involved in patient care. Brooke likes the idea of being immersed in all disease states and not limiting herself to one specific patient care area. She is especially motivated by the ability to apply evidence-based medicine to make a meaningful difference in patients' lives. Outside of pharmacy, Brooke enjoys reading, spending quality time with her family, and being outdoors doing activities that help her stay balanced.
Cameron MacGillivary, PharmD
Cameron is a current PGY1 resident at HCA Florida Northside Hospital. He is originally from Bel Air, Maryland where he went to Towson University and studied cell and molecular biology. Cameron then went on to earn his PharmD from the University of Maryland School of Pharmacy. Throughout pharmacy school he worked at CVS for 3 years and as an intern at The Johns Hopkins Hospital for 1 year. His passion for clinical pharmacy has motivated him to pursue PGY1 where he seeks to grow his clinical knowledge and prepare for a PGY2 residency. His current areas of interests include emergency medicine, internal medicine, ambulatory care, and critical care. In Cameron’s free time he enjoys playing golf, skiing, fishing, and watching the Baltimore Ravens and the Baltimore Orioles.
