For Medical Staff

We are here to assist you in the process of applying for medical staff privileges at HCA Florida Northside Hospital. Each Medical Staff Office and/or its designee performs medical staff support and credentialing functions for the HCA Florida Northside Hospital. This website will help you understand and begin the process of applying for privileges.


A pre-application is needed for credentialing medical and allied health professionals. Once a pre-application and completed delineation of privileges form has been received by the Medical Staff Office, the Medical Staff Office will screen to determine if the person requesting staff membership or clinical privileges is eligible before an application is sent to the hospitals Centralized Credentialing Center for further review.
