Mohammed Al-Said, PharmD, BCPS, BCCCP

Mohammed Al-Said, PharmD, BCPS, BCCCP is an advanced clinical pharmacist at HCA Florida North Florida Hospital with a focus in critical care. He is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Al-Said has extensive experience in hospital pharmacy and precepting, having served in these roles for more than 20 years. He received his Bachelors of Science in Pharmacy from the University of Jordan in Amman, Jordan, followed by his doctorate from Purdue University. He is dual board certified in pharmacotherapy and critical care pharmacy. He currently holds a position on the Residency Advisory Committee as the Education Coordinator.

Abigail Antigua, PharmD, MHA, BCCCP, FCCM

Abigail Antigua, PharmD, MHA, BCCCP, FCCM is an advanced clinical pharmacist with a focus in cardiac surgery, cardiology and critical care, and is the PGY1 Residency Program Director at HCA Florida North Florida Hospital. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Antigua has experience in teaching, precepting, research and hospital practice, having served in numerous leadership roles over her career. She received her doctorate from the University of Florida, and completed a PGY1 Pharmacy Practice Residency at the University of Florida Health - Shands Hospital, followed by a PGY2 Critical Care Residency at the University of Washington Medical Center in Seattle, WA. She is board certified in critical care pharmacy and is a Fellow in Critical Care Medicine (FCCM).

Kimberly Atkinson, PharmD, BCACP

Kimberly Atkinson, PharmD, BCACP is a clinical pharmacist at HCA Florida North Florida Hospital with a focus on ambulatory care and geriatrics. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Atkinson received her doctorate and completed her PGY-1 Pharmacy Practice Residency at the University of Florida College of Pharmacy with a focus in ambulatory care and academia. She currently serves as an Ambulatory Care and Geriatrics preceptor for APPE students and PGY-1 residents.

Casey Bardsley, PharmD, BCCCP

Casey Bardsley, PharmD, BCCCP is a clinical pharmacist at HCA Florida North Florida Hospital with a focus in emergency medicine. She is also a clinical assistant professor for the University Of Florida College Of Pharmacy. She received her doctorate from West Virginia University School of Pharmacy, and completed a PGY-1 Pharmacy Practice Residency at Carilion Roanoke Memorial Hospital in Roanoke, VA, followed by a PGY-2 Critical Care Residency at the same institution. She is board certified in critical care pharmacy and currently holds a position on the Residency Advisory Committee as the Networking Coordinator.

Stacey Bortlik-Hodgson, PharmD

Stacey Bortlik-Hodgson, PharmD, is a clinical pharmacist with a focus on emergency medicine and surgical services at HCA Florida North Florida Hospital. She is also a clinical assistant professor and preceptor for the University of Florida College Of Pharmacy. Dr. Bortlik-Hodgson received her B.S. in Pharmacy from Northeastern University Bouvé College of Pharmacy in Boston, MA, and her doctorate from the University of Florida College of Pharmacy. Her background and professional interests include pediatrics, patient safety, anticoagulation therapy management, academia, research, emergency medicine, perioperative care and military medicine.

Brent Carlson, PharmD, BCPS, BCCCP, BCIDP

Brent Carlson, PharmD, BCPS, BCCCP, BCIDP is an advanced clinical pharmacist at HCA Florida North Florida Hospital with a focus in infectious diseases. He is also a clinical assistant professor for the University of Florida College of Pharmacy. He received his PharmD from Lake Erie College of Osteopathic Medicine (LECOM) School of Pharmacy, then completed a PGY-1 Pharmacy Practice Residency at UPMC Hamot in Erie, PA, followed by a PGY-2 Critical Care Residency at Prisma Health Richland Hospital in Columbia, SC.

William M Chmely, PharmD, is a Transitions of Care Advanced Clinical Pharmacist for HCA Florida North Florida Hospital and the Medication Reconciliation Pharmacist Supervisor. Dr. Chmely received his doctorate from the University of Florida College of Pharmacy.

Chris DiRaimo, PharmD

Christopher DiRaimo, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus on general medicine. He is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. DiRaimo received his doctorate from the University of New England College of Pharmacy.

Shimaa Ghonim, PharmD, BCPPS

Shimaa Ghonim, PharmD, BCPPS, is an Advanced Clinical Pharmacist in Pediatrics and a Clinical Assistant Professor at the University of Florida College of Pharmacy. She obtained her PharmD from the University of Florida College of Pharmacy, and then completed a PGY1 Residency at UF Health - Shands Hospital with a focus on Pediatrics. She has 10 years of experience as Pediatrics Clinical Pharmacist and Preceptor of both PGY1 General and PGY2 Pediatric residents as well as fourth-year pharmacy students. She is board certified in Pediatric Pharmacy.

Josh Hawks, PharmD

Josh Hawks, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus in surgery. He is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Hawks received his doctorate from the University of Florida.

Andrea Koff, PharmD, BCGP

Andrea Koff, PharmD, BCGP, is the Director of Outpatient Pharmacy for HCA Florida North Florida Hospital. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Koff received her doctorate from Nova Southeastern University, and has more than 20 years of experience in inpatient and outpatient pharmacy practice and precepting. She is board certified in geriatric pharmacy and oversees pharmacy services for the Senior Healthcare Centers, including collaborative practice clinics and medication therapy management for geriatric patients.

Crystal Plasencia, PharmD, BCPS

Crystal Plasencia, PharmD, BCPS, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus in surgery. Dr. Plasencia received her doctorate from Nova Southeastern University and completed a Residency in Infectious Diseases and HIV/AIDS at North Broward Hospital District in Fort Lauderdale, Florida. She has additional experience in HIV medication management and research. She is also a clinical assistant professor for the University of Florida College of Pharmacy.

Nicole T. Reardon, Pharm.D., BCPS, FCCM

Nicole T. Reardon, Pharm.D., BCPS, FCCM is the Clinical Pharmacy Manager at HCA Florida North Florida Hospital. In her current role, she coordinates the clinical activities of the pharmacy staff, and participates actively in policy and protocol development and committee work throughout the hospital, including as Secretary of the Pharmacy and Therapeutics Committee. Dr. Reardon is a clinical assistant professor for the University of Florida College of Pharmacy. She received her Pharm.D. from the University of Wisconsin-Madison, then completed a PGY-1 residency at Oregon Health and Science University Hospitals and Clinics, and a PGY-2 Critical Care residency at the Nebraska Medical Center. Dr. Reardon is a board certified pharmacotherapy specialist and is a Fellow of the American College of Critical Care Medicine (FCCM).

Carl Smith, PharmD, BCGP

Carl Smith, PharmD, BCGP, is a clinical pharmacist at the Senior Healthcare Centers of HCA Florida North Florida Hospital. He received his doctorate from the University of Florida and is currently a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Smith is board certified in geriatric pharmacy, and specializes in ambulatory care and geriatric treatment.

Victoria Timmons, PharmD

Victoria Timmons, PharmD, is the Director of Pharmacy for HCA Florida North Florida Hospital. She is also a clinical associate professor for the University of Florida College of Pharmacy. Dr. Timmons received her doctorate from Purdue University, and has over 20 years of experience at several levels of hospital pharmacy administration, including as residency program director and as a Director of Pharmacy. Additionally, Dr. Timmons has been program faculty for Consultant Pharmacy courses, is an active member of FSHP and has previously served on the Board of Directors.

Vanessa Tomm, PharmD, BCPS, is a clinical pharmacist at HCA Florida North Florida Hospital. She received her doctorate from University of South Florida and completed a PGY1 training focused in pediatrics at Nicklaus Children’s Hospital in Miami, Florida. She precepts APPE students in general medicine.

James Winslow, PharmD

James Winslow, PharmD, is an advanced clinical pharmacist at HCA Florida North Florida Hospital with a focus on medication safety. He received his doctorate from the University of Florida and completed a PGY1 Pharmacy Practice Residency followed by a PGY2 Medication-Use safety and Policy Residency at the Clement J. Zablocki Veterans Affairs Medical Center in Milwaukee, WI. He has also completed an Advanced Fellowship in Patient Safety from the National Center for Patient Safety (NCPS) and Department of Veterans Affairs.

Kristen Martin, PharmD, BCCCP, is an advanced clinical pharmacist with a focus in critical care at HCA Florida North Florida Hospital. She currently precepts pharmacy residents and APPE students in cardiovascular ICU and medical ICU rotations.
