Crystal Plasencia, PharmD, BCPS

Crystal Plasencia, PharmD, BCPS, is the pharmacy operations manager at HCA Florida North Florida Hospital and has been a clinical pharmacist here since 2001. She serves as both a PGY1 and PGY2 residency preceptor. Dr. Plasencia received her doctorate from Nova Southeastern University and completed a Residency in Infectious Diseases and HIV/AIDS at North Broward Hospital District in Fort Lauderdale, Florida. She is also a clinical assistant professor for the University of Florida College of Pharmacy. 

Bickkie Solomon, PharmD, MBA-HM, BCSCP, CPEL, CSSBB

Bickkie Solomon, PharmD, MBA-HM, BCSCP, CPEL, CSSBB is the PGY2 Health System Pharmacy Administration and Leadership Residency Program Director for HCA Florida North Florida Hospital. She serves as both a PGY1 and PGY2 residency preceptor.  She is also an Assistant Professor at West Coast University. Dr Solomon received her doctorate from The University of Texas at Austin, her MBA – Healthcare Management from West Texas A & M University and Bachelor of Engineering (Communication) with Honors from RMIT University (Australia). Dr Solomon has completed an 18-month acute care administration residency equivalent program at Cardinal Health. She brings 12 years of broad experience in acute care/inpatient, outpatient, mail order, home infusion, correctional health, consulting, academia, pharmacy operations, durable medication equipment distribution operations and business development. Her expertise in specific pharmacy areas includes profit margin improvements, compounding, USP Chapters, regulatory compliance, accreditation, process and workflow improvements, change management, leading cross functional teams and projects, pharmacy licensing, medication safety, pharmacy system upgrades, training and development, and new products and services expansions. She has held Board Member roles at Rutgers University Customer Experience Program and Foundation for Young Australians as well as other leadership and ambassador roles for educational and not-for-profit organizations.

 

Education and Medication Safety

Josh Hawks, PharmD

Josh Hawks, PharmD, is an Advanced Clinical Pharmacist in Education, and is the PGY1 Residency Program Coordinator at HCA Florida North Florida Hospital. He is also a clinical assistant professor and the site coordinator for University of Florida College of Pharmacy students. Dr. Hawks received his doctorate from the University of Florida. Additionally, he is the Education Coordinator on the Residency Advisory Committee. 

James Winslow, PharmD

James Winslow, PharmD, is an advanced clinical pharmacist and the clinical pharmacy manager at HCA Florida North Florida Hospital. He serves as both a PGY1 and PGY2 residency preceptor. He received his doctorate from the University of Florida and completed a PGY1 Pharmacy Practice Residency followed by a PGY2 Medication-Use safety and Policy Residency at the Clement J. Zablocki Veterans Affairs Medical Center in Milwaukee, WI. He has also completed an Advanced Fellowship in Patient Safety from the National Center for Patient Safety (NCPS) and Department of Veterans Affairs.

 

Ambulatory Care and Geriatrics

Kimberly Atkinson, PharmD, BCACP

Kimberly Atkinson, PharmD, BCACP is an advanced clinical pharmacist at HCA Florida North Florida Hospital specializing in ambulatory care and geriatrics. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Atkinson received her doctorate and completed her PGY-1 Pharmacy Practice Residency at the University of Florida College of Pharmacy with a focus in ambulatory care and academia. She is board certified in ambulatory care pharmacy and currently serves as an Ambulatory Care and Geriatrics preceptor for APPE students and PGY-1 residents. 

Andrea Koff, PharmD, BCGP, MBA

Andrea Koff, PharmD, BCGP, is the Director of Outpatient Pharmacy for HCA Florida North Florida Hospital. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Koff received her doctorate from Nova Southeastern University, and has more than 20 years of experience in inpatient and outpatient pharmacy practice and precepting. She is board certified in geriatric pharmacy and oversees pharmacy services for the Senior Healthcare Centers, including collaborative practice clinics and medication therapy management for geriatric patients.

Carl Smith, PharmD, BCGP

Carl Smith, PharmD, BCGP, is a clinical pharmacist at the Senior Healthcare Centers of HCA Florida North Florida Hospital. He received his doctorate from the University of Florida and is currently a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Smith is board certified in geriatric pharmacy, and specializes in ambulatory care and geriatric treatment.

 

Adult Critical Care

Mohammed Al-Said, PharmD, MBA-HM, BCPS, BCCCP, CPh

Mohammed Al Said, PharmD, MBA-HM, BCPS, BCCCP, CPh is an advanced clinical pharmacist specializing in critical care at HCA Florida North Florida Hospital. He also serves as a clinical assistant professor at the University of Florida College of Pharmacy. Dr. Al Said brings extensive experience in hospital pharmacy and precepting. He earned his Bachelor of Science in Pharmacy from the University of Jordan in Amman, Jordan, and his Doctor of Pharmacy from Purdue University. Dr. Al Said is dual board-certified in pharmacotherapy and critical care pharmacy. 

Diamond Dumas, PharmD

Diamond Dumas, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus in adult critical care. She is the Research Coordinator on the Residency Advisory Committee. 

Kristen Martin, PharmD, BCCCP

Kristen Martin, PharmD, BCCCP, is an advanced clinical pharmacist with a focus in adult critical care and the PGY1 pharmacy residency program director at HCA Florida North Florida Hospital. She currently precepts pharmacy residents and APPE students in cardiovascular ICU and medical ICU rotations. Dr. Martin received her doctorate from Florida A&M University College of Pharmacy and completed her PGY1 training at HCA Florida West Hospital in Pensacola, Florida. She has also received her board certification in critical care pharmacy. 

Adult Internal Medicine

Jonathan Costro, PharmD

Jonathan Costro, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus on adult internal medicine. He is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Costro received his doctorate from the University of Florida College of Pharmacy.

Chris DiRaimo, PharmD

Christopher DiRaimo, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus on adult internal medicine. He is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. DiRaimo received his doctorate from the University of New England College of Pharmacy.

Holly Widell, PharmD, BCPS

Holly Widell, PharmD, BCPS, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus in adult internal medicine. She obtained her PharmD from the University of Florida College of Pharmacy, and then completed a PGY1 Residency at Winter Haven Hospital. She is board certified in Pharmacotherapy.

Ying Zhang, PharmD, is a clinical pharmacist at HCA Florida North Florida Hospital, with a focus on adult internal medicine. She is also a clinical assistant professor for the University of Florida College of Pharmacy. Dr. Zhang received her doctorate from the University of Florida College of Pharmacy. 

Transitions of Care

William M Chmely, PharmD

William M Chmely, PharmD, is a Transitions of Care Advanced Clinical Pharmacist for HCA Florida North Florida Hospital and the Medication Reconciliation Pharmacist Supervisor. He serves as both a PGY1 and PGY2 residency preceptor. Dr. Chmely received his doctorate from the University of Florida College of Pharmacy.

Tavish Tiet, PharmD

Tavish Tiet, PharmD is the transitions of care advanced clinical pharmacist at HCA Florida North Florida Hospital. He is also a clinical assistant professor for the University Of Florida College Of Pharmacy. He received his doctorate from the University of Florida College of Pharmacy.

Julia Mogilevskiy, PharmD

Julia Mogilevskiy, PharmD, is a transitions of care advanced clinical pharmacist at HCA Florida North Florida Hospital. She received her doctorate from the University of Wisconsin - Madison School of Pharmacy, and then completed a PGY-1 Pharmacy Practice Residency at HCA Florida North Florida Hospital. 

 

Emergency Medicine

Casey Bardsley, PharmD, BCCCP

Casey Bardsley, PharmD, BCCCP is an advanced clinical pharmacist specializing in emergency medicine at HCA Florida North Florida Hospital. She also serves as a clinical assistant professor at the University Of Florida College Of Pharmacy. She received her Doctor of Pharmacy from West Virginia University School of Pharmacy, and completed a PGY-1 Pharmacy Practice Residency followed by a PGY-2 Critical Care Residency at Carilion Roanoke Memorial Hospital in Roanoke, VA. She is board certified in critical care pharmacy and is the Networking Coordinator on the Residency Advisory Committee.

Stacey Bortlik-Hodgson, PharmD

Stacey Bortlik-Hodgson, PharmD, is a clinical pharmacist with a focus on emergency medicine and surgical services at HCA Florida North Florida Hospital. She is also a clinical assistant professor and preceptor for the University of Florida College Of Pharmacy. Dr. Bortlik-Hodgson received her B.S. in Pharmacy from Northeastern University Bouvé College of Pharmacy in Boston, MA, and her doctorate from the University of Florida College of Pharmacy. Her background and professional interests include pediatrics, patient safety, anticoagulation therapy management, academia, research, emergency medicine, perioperative care and military medicine.

 

Infectious Diseases

Brent Carlson, PharmD, BCPS, BCIDP

Brent Carlson, PharmD, BCPS, BCIDP is an advanced clinical pharmacist at HCA Florida North Florida Hospital with a focus in infectious diseases. He is also a clinical assistant professor for the University of Florida College of Pharmacy. He serves as both a PGY1 and PGY2 residency preceptor. He received his PharmD from Lake Erie College of Osteopathic Medicine (LECOM) School of Pharmacy, then completed a PGY1 Pharmacy Practice Residency at UPMC Hamot in Erie, PA, followed by a PGY2 Critical Care Residency at Prisma Health Richland Hospital in Columbia, SC. Additionally, he is the Medication Use Evaluation Coordinator on the Residency Advisory Committee. 

 

Neonatal Intensive Care Unit

Shimaa Ghonim, PharmD, BCPPS

Shimaa Ghonim, PharmD, BCPPS, is an Advanced Clinical Pharmacist in Pediatrics and a Clinical Assistant Professor at the University of Florida College of Pharmacy. She obtained her PharmD from the University of Florida College of Pharmacy, and then completed a PGY1 Residency at UF Health - Shands Hospital with a focus on Pediatrics. She has 10 years of experience as Pediatrics Clinical Pharmacist and Preceptor of both PGY1 General and PGY2 Pediatric residents as well as fourth-year pharmacy students. She is board certified in Pediatric Pharmacy.

 

Oncology/TCT

Ashley Richards, PharmD, BCOP

Ashley Richards, PharmD, BCOP is an advanced clinical pharmacist for the Transplant and Cellular Therapy (TCT) Program at HCA Florida North Florida Hospital/Sarah Cannon TCT Network. She earned her doctorate from the University of Florida, completed a PGY1 Pharmacy Practice Residency at Shands Jacksonville, and followed by a PGY2 in Hematology/Oncology Pharmacy at the University of Washington/Seattle Cancer Care Alliance/Fred Hutchinson. She has been a TCT pharmacist for more than 15 years. Her interests are acute leukemia, myelodysplastic syndromes and multiple myeloma, and many other supportive care topics within the hematology/oncology field. She serves as both a PGY1 and PGY2 residency preceptor.
