Many insurance plans require pre-certification before admission. To avoid reduced benefits, call your insurance company to verify your coverage and make sure your authorizations are in order. If you have any financial questions, please call our financial counselors at (813) 634-0173 or (813) 634-3301, ext. 1033, weekdays from 8:30am to 5:00pm.

Outpatient testing

Make sure your doctor has called the hospital to order your procedure. Or, if the doctor gives the order to you, be sure to bring it with you when you report for testing.

Medical records

We are committed to maintaining the privacy and confidentiality of all patient records. If you need a copy of your medical record, please call (813) 634-0167. We are pleased to serve your medical record requests.
